UTICA — North Fork Local Schools will hold its annual Kindergarten Registration Days on Wednesday, May 9 (Utica Elementary Day), and Thursday, May 10 (Newton Elementary Day) at the Utica Church of Christ located at 115 North Central Avenue in Utica. The registration will begin at 8 a.m. and run until 2:30 p.m. both days. Appointments are necessary. Students may schedule for either day; however the staff they see will all be from the appointed school of the day.
Children whose birthday is on or before Aug. 1, 2007 are eligible to attend. The registration day gives both parent and child an opportunity to meet many staff members and community volunteers from the district. While parents complete the necessary paperwork an escort accompanies each child to the following areas: Hearing, readiness concepts, kindergarten teacher, vision, speech and language, brief dental exam, brief optometric exam and brief physical exam. An optometrist, a dentist, and a nurse practitioner or physician will provide these free exams.
Once paperwork is completed, the parents are invited to an informational meeting featuring the principal and the guidance counselor. The Utica Police Department provides safety fingerprinting for the children. A school bus will be on hand for the students to tour and the transportation supervisor will be there to answer questions.

Items to bring to screening are:
•Copy of child’s birth certificate that the school will keep.
•Two copies of the immunization (shot) record for child.
•Copy of official custody document from the court (if applicable).
•Copy of child’s Social Security card .
•Urine sample in a clean jar with child’s name on it.
•Child should wear comfortable clothing.
•Health history form (obtained from the North Fork website).
•Completed registration form (obtained from the North Fork website).
•Transportation form (obtained from the North Fork website).
•Copy of proof of residency in the district (eg. Utility bill).
•Free and Reduced Lunch application (obtain from the North Fork website).
•Five DPT (diphtheria, tetanus, pertussis).
• FourPolio (last one must be after the fourth birthday).
•TwoMMR (measles, mumps, and rubella).
•Three Hepatitis B.
•Two Varicella (chicken pox).
